Billing Medicare for Gradient Compression Stockings

How are Durable Medical Equipment dealers billing this to Medicare? I am told Medicare rejects the claim if diag is not right so the provider cannot get an eob in order to submit it to state medicaid? Codes cannot be made Medicare exempt if Medicare does pay sometimes...if anyone has any input it is greatly appreciated. Thank you.
