Annual wellness visit and hearing testing

Can we use the OAE (otoaccustic emissions) testing for everyone during the medicare annual wellness visit as a screening test and bill separately for that? I believe we are supposed to screen for hearing loss during the AWV/ G0438 visit and subsequent years

CPT® 92558 was created to describe automated OAE screening. CPT® 92558 should be used when an automated pass/fail screening is performed, via a fixed number of frequencies at a single intensity level, when administered by support personnel, an audiologist, or a physician. This procedure has been designated by CMS to be a non-covered service under the Medicare program.

So when a pt comes in for AWV, G0438, screening is allowed? How would I code G0438 with EKG?

The only time an EKG is covered by Medicare is with the IPPE, this is a separate service from the AWV. Bill out the EKG with the appropriate G code (G0403, G0404, G0405) – when it is a screening EKG as a result of a referral from an IPPE.

93000 can be billed out with the AWV G0438/G0439 but needs to have a dx that indicates medical necessity as an EKG with a diagnosis of V70.0 that may have been sent in conjunction with the G0438 will not get paid – Medicare doesn’t cover preventive services meaning EKG with dx V70.0. It will definitely deny as inclusive. The diagnosis code must support the EKG with symptoms or medically necessary diagnosis.
The following are included in AWV:
Medical and family history
– List of current medical providers
– Height, weight, BMI, BP and other appropriate routine measurements
– Detection of cognitive impairment
– Review risk factors
– Review of functional ability
– Establish a written screening schedule for next 5-10 years
– Establish list of risk factors
– Provide advice and referrals to health education and preventative counseling services
– Other elements as determined by the Secretary of Health and Human Services
