Preventive Services related to Affordable Care Act for 2011

We have seen a lot of large insurance carriers bundle agressively since the new regulations went into effect on preventive services for 2011. The biggest problem has been the processing of E/M services at the same time as the PE.

What is the most worrisome is that insurance companies are (on some claims) dropping the PE CPT® altogether, and only processing the E/M service which then incurs a copay/deductible for the patient.

We have also seen things like EKG, Bone Density, Shots, etc. being bundled as part of the PE service despite us using correct modifiers on the claims.

Has anyone seen this happening to their claims. It seems to be in some related to new edits the insurance companies have put on their systems.
