Are compounded drugs billed with J3490? If not, please provide instructions. Thanks

Yes, compounded drugs are billed with J3490 or J9999 because they have no NDC.
Per Part B instruction' We billin for a compounded drug, the invoice information can be put into item 19 of the 1500 claim form or ehe electronic equivalent. Providers should indicate te drug is compounded,the name of the drug, the dose and the invoice price plus sipping for each line item'.
