hospitalists discharge to primary care

Hi - I'm working with a large employed physician network. When their hospitalists discharge a patient and they are seen later for follow up by a PCP (that they've never seen before) who is also employed by the same network, are they considered a new patient to the PCP?

If both are Internists: established.
