pain pump reprogramming and refills

Cancer patient coming to pain clinic with worsening condition to have pain pump reprogrammed each day--reprogrammed 7/5/11; 7/6/11; 7/7/11; and 7/8/11. Pump inserted 6/17/11 so we are past global days for procedure 62362. Is it appropriate to bill 62368 each day or would it be better to charge the appropriate level E/M visit? Would it also be appropriate to bill 62368 with 95991 when pump requires refill or is the reprogramming included as maintenance of the pump? Thx.
