QW Modifier

Do I need to report QW Modifier with Medicare HMO payers or is that required only for direct Medicare alone?

CLIA waiver testing is not restricted to Medicare only. I have all CLIA waiver tests set up to automatically populate a QW on all claims, no matter the payer and have never had a problem.
