Closure of Abdomen 25 cm and Partial Omentectomy of Damage control lap

HI,
Surgeon performed distal pancreatectomy with splenectectomy -48140 and adrenalectomy 60540. patient was left open...do I need to append mod 52?

then, patient return to OR for removal of perihepatic and perisplenic packing, washout of abdoment, partial omentectomy and closure of abdomen.....Do I just code this as 13160-58 or I can also include 49255-52?

Please advise.
