No offense, but this question is extremely vague.

if I understand your question correctly, the 'level of MDM' (Medical Decision Making) required is predicated on the E&M code used. The 99212 code requires 'different'/fewer items than does 99215. With each move up, more information and validation of MDM is required.

also, I've used this website and form (even to building our own 'audit form' off of this AACAP form) for auditing, training, and support of those attempting to learn the different 'levels' of validating information required of each different E&M billing code - which is much more difficult in the Mental Health world.
