Wellness exam and I&D, what modifier to use?

A patient came in for her yearly physical and also had a sebaceous cyst removed. I orginally billed the claim as a 99396 using icd-9 code V72.31 and a 10060 with modifier 59 using icd-9 code 706.2. The I&D was paid but the physical was denied. Do I need to add modifer 59 to the physical or use a different modifier?
