Colonoscopy and Polypectomy

When my doctor does a Polypectomy by snear also hot biopsy forceps I always codes 45385 45384 using 59 for the 45385 and 45 for 45380. But some insurances are requesting refund for the Colonoscopy 45380, I just don't know what I am doing wrong? Please help. Thank you.

Both CPT® codes 45384 and 45385 include biopsy services and are colonoscopy services. They can be billed together, assuming that two different approaches were done. In this instance, if you have a removal by hot biopsy and a separate removal by snare during the same session, you can bill 45385 and 45384, using the 59 modifier as you described.

It's unclear why you are billing 45380. Is there a third cold biopsy being done during the same session?

Are you saying you are putting a 45 modifier on 45380. That is incorrect you should be using a 59 if he is removing a polyp by cold biopsy method.

I would agree on that point as well, with modifier 59. I just didn't see anything in the original post suggesting a cold biopsy polyp removal was happening.
