Rtc tear

my Dr preformed 29824,23405 and 29822. I received payment for 29824 and 23405 but 29822 was denied. Can all these codes be billed together or maybe I used the wrong modifiers (51) thanks for your help

29822 is bundled to 29824. modifier 59 would be used ONLY if the debridement is done at a separate stie other than the 29824. routinely limited 29822 is always bundled and Medicare won't consider it.
