Normal First Pregnancy vs Other Normal Pregnancy

I am trying to decide when one should use V22.0 vs. V22.1

V22.0 is Supervision of normal first pregnancy.
V22.1 is Supervision of other normal pregnancy.

The opinion of some in my office is that no matter how many times a patient has been pregnant, until the patient has had a term delivery, any pregnancy is considered her first. They then insist V22.0 is appropriate.

The other opinion is only a G1P0 patient should be coded with the V22.0. For patients with G2+, P0+, V22.1 should be used.

Which scenario is correct?

I disagree with the others in your office, the ordering of the words matters. "Normal first pregnancy" implies a patient's very first pregnancy that is also normal, not a subsequent normal pregnancy after a high risk pregnancy or premature birth. If it said "first normal pregnancy" that would imply a patient's first pregnancy that qualifies as "normal" regardless of whether it's the first time the patient has been pregnant. Since it's the first wording, it clearly means it's the patient's first pregnancy ever, and it is also "normal." If the patient has ever been pregnant before (including pregnancies that ended due to miscarriage, abortion, or premature birth), you would use V22.1. The description is about the previous and current pregnancies themselves, not the birth outcomes. A pregnancy that ends early could still have been completely normal up until it ended even if it didn't result in a live birth. I have a degree in English literature, for what it's worth.
