Pump Billing/DMERC

5FU is loaded in a pump for continous infusion over a period of multiple days, should this be billed as DMERC. All commerical payers are denying the 96521 as incidental to the 96411 or other administration codes.

The patient does receive 5FU in the office with other chemotherapy drugs. How do we bill the pump refill that the patient goes home with?

We use 96416-59 for chemo pump initiation. The CPT® book says 96521-96523 are for non-chemo therapeutic drugs.

Additionally, we do not supply the pump itself. We get them from a supplier and fill them here.
