Anesthesia code for facet injection

If anesthesia is required for a facet injection what anesthesia code can be used? My 2011 crosswalk published by american society of anesthesiologists has no anesthesia code for 64490. We have several pain management doc's who schedule their procedure's in the hospital and the CPT® code 64490 states: anesthesia care not typically required. Also there are several ins carriers that do not allow anesthesia for several pain mgt procoedure, we are in nj
