Billing Proc Code 26615

Good Morning,
I am still waiting on your help in billing Proc. Code 26615 4th and 26615 5th to Caresource Ins. My question was should I used the 51 modifier or 59 modifier since it's not taking the modifiers F8 and F9.

The Doctor did both procedures. Please help me to bill this claim to Caresource.
