RFA and Biopsy billing

Guys,
Can I bill for an RFA and a Bone Biopsy at same session. Carrier paid for the biopsy and denied the RFA. They don't come up as bundled, and it doesn't say anything to the contrary in my book, but i think i remember reading somewhere that unless the biopsy is for confirmation of lesion, etc. at time of service, it's not separately billable.
Can I get some input on this.
