Routine or Diagnosit Hearing exam

Do you code by presenting complaints, signs and symptoms or do you code by results?
Some scenarios are,

1. Patient presents telling us they are having no problems, just want to establish a hearing baseline because they are now xx years old. We test and find there is some hearing loss. Routine visit? or no turned diagnostic due to findings? (I would likely code diagnosis this V72.19 primary and 389.18 or other HL code secondary.)

2. Patient presents complaining of noticeable problems. We test and confirm significant hearing loss. Routine visit?

In the above, I am thinking of this as if this is their first hearing exam, it is a screening/routine, once problems are found/finally diagnosed, each visit after that is a non-routine/diagnostic. Am I correct?
