Diagnosis codes

I've been getting various answers- which diagnosis code should go first when we are entering charges, as in a cold and hypertension. Thanks

diagnosis

the diagnosis code assigned as primary should be "why" the patient made the appointment. If the patient presents with cold symptoms and a diagnosis of Upper respiratory infection is made, that is primary. If hypertension is also managed, it would be a secondary code.
