Billing a physical and cerumen removal same day

I billed a 99212 with a 25 modifier a cerumen H61.23 and 69210 then a 99396 with a Z00.00 all in one sequence and was only paid for the 99212 and 69210. the 99396, 93000 and 81002 were all denied. Please help.
