Level 4 office visit. Is there an easy list available

I am needing a list to give to my doctor to explain each level of office visits. I have two physicians that I work for. Both are under same tax id number. One of the physicians " DR. R" is no longer taking UHC. Some of our patients are wanting to see the other physician "DR O". Please note these are established patients with our office and they are coming in for the same or original dx they are just now seeing "Dr. O" rather than " Dr. R" due to him being NON PAR with UHC. Dr. O is wanting to know why we can not charge a new patient visit b/c they have not been seen by him and his visit with patient is taking longer since he is going over everything b/c he is seeing them for the first time. Ok so I am long winded.. lol.. Does anyone have a website or an easy guide on how to bill each level? any questions please contact me at wventssb@yahoo.com. i would show them that. Page 4 second paragrapgh explians it really well. There should be no question after seeing that. And as far as guidance I would go to your local MAC website and pull p the DG's for 95 or 97 and any helpful tips the local carrier offers.
