Supervision (g0181) denied on b-5 :coverage guidelines were not met or exceeded.

Regularly facing a problem on supervision like B-5 coverage guide lines were not met or exceeded. For the most of the claims we are billing in correct procedure. Below is the following procedure.

First we are billing face to face , then re certification. between the recert and recert taking 60 days gap. In this between gap billing two supervisions each one end of the calender month.

Still getting the same reason denial. Is there any more action needed to resolve this denial please explain.
