billing heart caths and stents same day same session

Is anyone else receiving rejections from insurance companies regarding the billing of a heart catheterization and a coronary stent/angioplasty on the same day at the same session?
We normally bill a coronary stent (92980) with the apprpropriate artery, and a heart catheterization (93458) with a 26 59 modifier. Ever since the cath codes changed, I dont believe the CCI edits changed. Can anyone help me with the proper billing on stents and caths?
