nursing home visits

So our doctor made a "house call" to see one of our Medicare patients. She lives in the special care (Alzheimer's) unit of a local nursing home. This is her permanent residence and has been for years. I used code 99309 for the visit. I first used place of service code 12 (home), and used the nursing home address for the facility info. Medicare denied payment saying the procedure code is inconsistent with the place of service. I changed the place of service to code 13 (assisted living facility) and got the same denial. Any suggestions?

Nursing Home POS is 32. This is probably what you need.
