Coding for a pediatric International travel exam

Help!!!! Can someone please advise on the PROPER way to code (E/M and DX) a pediatric exam for international travel, where vaccines, medications & prophylaxis treatments are discussed during the exam?

Is the E/M just a 9921X, or will I also need to add a 99402 (with mod 25 on E/M)?

On the dx code, z71.89 & z02.89, are not descriptive enough...

Please advise, as my head is spinning from endless research & coming up empty!!!!
