Preventive Medicine & problem-oriented ICD-9 codes

Hello

Is it appropriate to assign V70.x and any other documented problem-oriented ICD-9 codes (493.90, 300.00, 786.5) as secondary, tertiary, etc diagnosis?

Also in audit a preventive medicine note...is there a specific number of elements (within hx & exam & counseling) that will suport "comprehensive"

Hx: past illness, surgery, meds, allergy, famhx & socialhx, status of chronic conditions ---- do I need to see all or 1/2 to qualify?

Exam: Do I need to see all Organ Systems/BA or 1/2 to qualify?

Counseling/Guidance: nutrition, physical activity, healthy wt, injury prev, misuse tobacco, drug/alcohol, sex, STD & contraception, dental health, immunizations & screenings ----- Do I need to see all this mentioned or some if it?

Both the American Academy of Child & Adolesent Psychiatry (AACAP) _and_ the National Council have developed information and examples of "how many" of "what" type of data and system review is required for each level of E&M coded session - which, also, distinguishes on whether one codes and bills by time or by complexity of the services provided during the Clt/Pt encounter.
The AACAP website [www.aacap.org ] has examples of notes which include info on how to validate the coding chosen, a 'Coding Algorithm', PwrPt slides that can be accessed, and a 'summary guide' of "E&M Services Guide: Coding by Key Components". Once you reach the website, just type in E&M Coding into their site search, and you should see all these options. While this will not answer your question regarding ICD codes, I do believe it may answer some of your other questions. Good Luck.
