BCBS of Alabama and Medicare Bundling Issue

How do I charge BCBS of Alabama and/or Medicare for a new patient E/M, x-rays, trigger point injections, and medications used for injections and get paid for all services instead of just the injection and medications. I don't understand how the insurance company expects the doctor to know what kind of treatment the patient needs without examining the patient or taking x-rays..if needed.

As long as your documentation supports the services billed and the appropriate CPT® codes, modifiers, and diagnosis codes are used, you should get reimbursed for all of it. If you would like to send me an example to look at I would be more than happy to give my opinion.
