billing questions for home pt/inr

Hello,

How would an Independent Diagnostic Testing Facility bill for G0249 to Medicare?
Can the Place of Service 81 be used? What date of service is to be used? Would it be the date of the 4th test since the IDTF can bill once for every 4 tests? Any help would be appreciated. thank you
