proc. Caode 77002 was done in the hospital as out patient service and was bill to wellcare with 26 moadifier and tc modifier and was denied by wellcare . Can you tell me what I did wrong should I not have used the tc modifier.

The hospital bills the technical component on UB form. The physician bills -26 on code on 1500 claim form.
