E Codes

I work in a family practice and wondering about using E codes from the ICD 9 book are they usually used in the hospital setting only? Because a claim went out that a poster used along with a regular injury code and the medicare claim rejected.

E codes are descriptors to be used secondary to the injury code. What was the rejection message from Medicare?
