has anyone ever heard of a carrier asking for a modifier for 99354? i know modifer -21 was deleted in 2009 because not everyone recognised it or used it.

No, but make sure another code (e/m) was billed with it for the same dos. This is an add on code and cannot be billed alone.

yes it is an add on code and was billed correctly. i just think they are being stupid and trying to figure out a way to fight it.
