What to code when a Hospitalist follows a patient to another facility

If a Hospitalist (Primary Care or Specialist) is following a PT in an acute care facility and the PT is transferred to a step down facility (Rehab for example), does the provider bill for a subsequent hospital visit (say 99232) or an initial hospital visit (say 99222)?
