hypertension coding

A patient with hypertension came to office with symtoms which is not related to hypertension but the doctoer documented hypertension. In this case, do I assign hypertension ICD-9-CM code in addition to the code for symtoms?

did the physician just list it on the problem list or list it in the assessment?

If in the assessment: Hypertension, good control, continue current meds.

Or even: hypertension: no changes to plan

then I believe the physician addressed the condition at the unrelated visit (which is not unusual) and I would report the hypertension as the second diagnosis on the claim form.
