Subsequent Codes used for Discharge

I was hoping someone could help me with a question about discharge billing. I have a physician that documented that they had spent a total of 120 minutes in coordination discharge management.
Can a subsequent E/M code along with a prolonged service code be reported based on the extensive time documented by the physician vs a 99239 discharge code?
