modifier 59 usage with code 94150

When can we add modifier 59 with 94150? I used this code along with 3 other codes like 96401, 94640 and J2357 and received denial for 94540 , with reason like it's an inclusive service. The CCI edits do not bundle any of these among each other. Is this because of the presence of the code 94640? Or shall I report all the codes without modifier 59? Kindly help.
