Billing for a contracted CRNA

My Pain Management Doctor contracted with a CRNA who is out of network with all carriers and we need to know how to bill her servcies to the carriers. Should we list the CRNA anywhere on the claim such as in box 31 or 24J as the rendering provider? Our Pain Doc will be supervising her? We just want to make sure we are billing correctly. We bill mainly spinal cord procedures.
