Chemo drug infusions

Is it appropriate to combine IV infusion times from 2 different chemo drug infusions? For instance, Avastin is infused for 1 hr. 30 min. and Oxaliplatin is infused for 2 hrs. 8 min. Would the appropriate coding be:

96413, 96417, 96415 0r

96413, 96417, 96415x2 (combining the 30 min. Avastin with 8 min of Oxaliplatin) Or

96413, 96417, 96415, 96409 (for the remaining 8 minutes for the Oxaliplatin)
