Cardiology coding

Whenever Echocardiography and hospital E/M services are billed on the same day or Nuclear studies and hospital E/M are billed on same day, only E/M gets paid and Echo/Nuclear is denied as inclusive with E/M

Please advise correct billing/coding guidelines.

Did u add Modifier 25 with the E/M service code. We are billing the same services and getting paid without any issues. The only thing is we should add modifier 25 with the E/M..
