93015 Stress Test

My office is billing 93015 with place of service 22 (outpatient hospital), to Blue Care Newtwork. These claims are being clinical edit with QNT and reason 4 = procedure code is inconsistent with the modifier used or a required modifier is missing. NO MODIFIER was used, and modifier 26 is inappropriate. Can anyone help.

93015 is for all three components of the stress test, and if you are performing in an outpatient department, the facility should be billing the technical component 93017 with no modifier. The physician would bill 93016 for the supervision, and 93018 for the interpretation and report.
