Rule out

I am new to coding and trying to keep all the rules straight. So if you have a dx of F/U R/O MRSA on a lab test would you code the MRSA or query the physician for a more specific dx?

Do not code diagnoses documented as "probable", "suspected", "questionable", "rule out", or "working diagnosis" or other similar terms indicating uncertainty. Rather, code the condition(s) to the highest degree of certainty for that encounter/visit, such as symptoms, signs, abnormal test results, or other reason for the visit.
