CPT® 75630 inclusive

CPT® 75630 with MOD 26, 59 denied stating inclusive with CPT® 99253-25, please advice what should i do on this.

why are using modifier 59, was another procedure done w/the sono? if not I would advise in taking that off. you already have the modifier 25 w/the office visit, so i don't know why you would of added the 59 on the procedure.
