Transitional Care codes

I am confused as to when to use the 99495 and 99496 transitional Care Management codes. Currently patients discharged from the hospital followup within a few days at the office. Dr. has me billing the these codes.
recently Humana denied this and said that these codes are to be billed after 30 day of the discharge. Can anyone direct me to information that would explain this clearer for the exact use and when to use them. Thank you
