Artificial insemination

If a specimen is obtained in the office by the spouse, then washed under high power evaluation on the microcope, there are numerous normal appearing sperm with forward motion, specimen was divided into small Pipelles and inserted into the uterus without difficulty. Can this be reported with codes 58321 and 58323? What code would be reported for viewing the sperm under the microscope or would this be included in 58323?

Insemination

yes, codes 58321 and 58323 can be billed together because both services were performed. Viewing the sperm is not separately billed

Do you bill 58323 under the female? And what ICD-9 code is appropriate?
