How to bill 64484 done bilateral and with two units

Can anyone tell me the correct way to bill this? I am having a difficult time with getting this paid I billed L3, L4 and L5 transforaminal epidural injection under fluoroscopy

64483 50 x 1 unit (pays)
64484 50 x 2 units denies

and for unilateral L3, L4 and L5 transforaminal epidural injection under fluoroscopy

64483 LT x 1 unit (pays)
64484 LT x 2 units(denies)

64483 is your base code
64484 is your 2nd level at 50 (for bil) , and bill 1
64485 is your 3rd level and add'l levels at 50 (for bil) , and bill 1
