Anticoagulation

Hello and thank you in advance for any feedback you can provide.
NP billed 99251, dx 715.96, 790.92, V58.83, V58.61 & 530.81

NP documented: Anticoagualtion Clinic POD #1 s/p L TKA Denies C/P, SOB, calf pain CV: S1 S2, rate regular RESP: lungs CTA B/L ABD: soft n/t, +BS EXT: + pedal pulses, no edema\INR: 1.0 today. Coumadin 7.5mg PO tonight PT, INR in AM

Based upon above I am changing to 99231, Dx V58.83, V58.61 Incomplete history. No HPI. Gave s/p TKA credit to PFSH Question 1. Would you agree with above? Question 2. No clear CC. Would you give credit? Especially since it is an anticoagulation clinic? Question 3. INR 1.0 reflects a normal range, so Dx 790.92 would not be appropriate. Do you agree? Question 4. Can V58.83 be used as a primary diagnosis code. I usually look for "PDx" designation to guide me but this code does not have.
