We have a new physician at our practice and his patients are following. Will we bill new or establish patient.

Established, if they were seen within three years.

Look at the definition and flow chart in the front of the CPT book. Since the patient had a professional service from the physician, (assuming in the past three years) it is an established patient, even though group and tax ID changed.
