CPT® 76700 vs 93975

Hi We need clarification regarding billing of CPT® 76700 AND 93975 most of the insurance denying the claim for CPT® 76700 when billed along with 93975 is there is any guideline to follow for these procedure please advise and help me.

The codes are bundled. Are you using a 59 or one of the XE,XP,XS,XU modifiers if appropriate.
