Rapid strep test denial

Can someone please let me know if I am incorrectly billing for a rapid strep test.
Doc bills 87880 with a -QW modifier. Medicare recently started sending claims back stating doc is not authorized to bill for this CLIA waived test. Doctor has up to date CLIA number and all information is in order. I am at a loss as to why they are denying this service. Any ideas/help would be appreciated.

This issue has justed started within the last month . We have always billed the same way for this waived test.

I have had that denial in the past. It seems my software vendor made some changes and my docs CLIA # was not being transmitted to Medicare.
