transition of care

Medicare is denying payment for TOC because "Time frame requirements between this service/procedure/supply have not been met." According to the rules we bill 30 days after discharge. Pt was discharged on 5-29-13 and we billed 99495 on 6-29-13, which was denied for the above reason. What am I doing wrong?

Day of service should be 06/27/2013. Starting ON day of discharge, count 30 days...
