Coding a colonoscopy with an endoscopy

I have a procedure that we coded, and I wanted to see if there was anyother way to code.

DX: 569.3, 535.50, 530.81, 211.11, 787.20 and 553.3

PROC: 45378
43239 MODIFIER 51
43251 MODIFIER 51

Humana disallowed the 43239, is there a way that this would not be allowed. Thanks.

Well, it denied because you can't bill 43251 & 43239 together without a modifier. I don't think -51 is appropriate. Did you try -59? Do you have documentation to support both these codes? Did the doctor really biopsy and then snare during the EGD?

Should the 59 go on 43239 or 43251?
